Fix for #1901090: For /workflow-dashboard to work need to tick "Show a comment field on ... workflow tab form".

Show title with Single-Action buttons.
Fix bugs regarding transitioning of workflow via the Edit tab (as opposed to View and Workflow tabs).
More sensible redirect for cases where the action isn't initiated from the the node form.
Fix permission to transition from Edit form.

Working on integration with Node Relationships (with modal view).

New View and block "Workflow History" to view state transition history for all nodes or a single node (context sensitive).
This View contains "Edit comment" links that allow privileged users to change or erase workflow log comments.
Workflow extensions configuration options tidies up and new option added to control whether workflow log comments may be blank.

Fix for #1097328: Menu changes are not saved

Now features three UI styles for changing workflow state: radio, drop-down, single-action.
All of the above available as a configurable block that may be put anywhere on the page.
Workflow form title configurable, optionally using tokens.

Fixed #972858: Workflow Comment Form Removed and #970752: Undefined index: workflow in workflow_extensions_form_alter()
