After I add my first custom link, then go back to add another, there is no "add" button that allows me to add any more links... only the existing link I just added, with "edit" and "delete" links.
I found that if I append "/add" to the url, then I'm taken to a screen that lets me add additional custom links.
(changing http://www.example.com/admin/settings/xmlsitemap/custom to: http://www.example.com/admin/settings/xmlsitemap/custom/add)
Maybe my style sheet is interfering with the appearance of the "add" button?
