tested this on two separate sites (one running D6.17 and the other running D6.19). When I go here:

/admin/content/nodewords/custom/add

I get a blank area under the main tab navigation. See attached grab. The "Meta tags for custom pages" module is enabled.

Make sure you have the desired meta tags enabled on admin/content/nodewords under the 'Meta tags enabled on administrative settings pages' fieldset.

There should definitely be a warning displayed saying "You don't have any meta tags enabled for administrative settings pages", with a link to the nodewords general settings page.
