You have missed a lot of checkpoints on how to do a correct application for a CVS account.
So please read through this carefully http://drupal.org/cvs-application/requirements and all childpages and pages that it links to and correct your application. You could also look at other applications to get a hint about what you have missed.
Until then, I will not review your code and probably no others either. There are others that have spend more time and done a great CVS applicaiton that deserve reviews more then you in this case.
ex.
You have written one sentence to describe the module, other writes a lot more.
Your issue title is wrong.
