I'm getting

SQLSTATE[00000] [1130] Host 'example.com' is not allowed to connect to this MySQL server.

See the attached images for the various commands I've run.

Does MySQL need a root@% record? If so this doesn't exist by on the servers I'm looking at, so maybe this should be added to INSTALL.txt.

I worked out the problem, I'll add some info to the troubleshooting docs.
