Breaks anything to do with ImageCache, obviously, but I would add a warning to the project page to protect users.

Please give us more detail information.
I'm using Imagecache and this module together with several sites and I do not see any problems with imagecache. Actually, as default, this module would not touch any anchor URLs to image files (jpg, gif, png) unless you intentionally add them to the supported file extensions. So I can not understand why it breaks imagecache.
