Improve reply forms

Right now AF doesn't do pretty much anything with the reply form. It would be good to differentiate it from the quick reply at the bottom of the topic. Some thoughts:
- Strip down comment form at bottom to just the basics
- When on other page, have smileys, help, and more stuff
- Make sure NC's topic review is fully compatible with AF
- Turn log field into "edit reason" since it's used that way but isn't clear to end users.
- Make this optional in settings because some people may already be doing this on their own and we don't want to clobber it.
- Maybe make a template file for the form page
