#990888: notice: Undefined index - by anon - fix PHP notices.
Fix minor XHTML validation errors. Add prefix to classes to prevent numeric only classes.
Other changes
#644130: Unexpected or even wrong scaling of videos - by Pisco - changed how scaling of videos work to be more consistent, introduce control_bar_height for codecs and the ratio parameter for users. This is the main reason for the beta release. Needs testing.
