I can upload some images via IMCE, but for some I get "Upload failed." I can't figure out what might be causing the failure on these particular images (not size or type.)
When I go to the following page, I get "Access Denied"

This is happening with different users logged in (including user 1), on different workstations and with different images (all of which are small) and file types (gif, png, jpg), and whether or not creating a thumbnail.
I have verified that the files directory and all sub-directories are writable by anyone, the file doesn't currently exist on the server, the IMCE settings allow for all file types and file sizes, etc. All it gives is the "Upload Failed" message.
Drupal 6.20, IMCE 6.x-2.1, IMCE Wysiwyg bridge 6.x-1.1, Wysiwyg 6.x-2.3, PHP 5.3.6, Windows Server 2008, IIS7.
