3rd level menu disappears on some pages

Hello and thank you for the great module.

The website I'm working on is experiencing some strange issues I've been struggling with for some time now. In short, some pages menu block works, on some pages it doesn't. Both working pages and not working pages seem to have the same configuration. I attached screenshots for your viewing pleasure.

I tried to remedy this by installing old version of the module, trying various patches I found in various issues here, and seemingly every menu_block configuration possible to bring the menu back.
