line 778 of sites/all/modules/hierarchical_select/hierarchical_select.module on some node edit pages. Looks like $names in hierarchical_select_after_build() is returing zero sometimes thus $hs_form_build_id is undefined. Not sure about the correct way to fix this so I'm waiting for guidance on this one.
