Patches for handling of extras, date calculation and more [#1356896]

Please, add the appropriate

configure = admin/config/people/legal

line into the .info file.

I rolled this and #1356886: .DS_Store, plus a couple of small fixes into a patch.

Changelog:

removed .DS_Store files
added 'configure' in .info
changed date calculation to use Drupal's own localized and user customized function
removed the Preview button, it did nothing
fixed the handling of Extras, it is now flexible, growing and shrinking in number
fixed the original order of Extra checkboxes
