Single conditional field checkbox

Hi,

I want to create a required checkbox that appears only when a selection from select list is made it then asks if the user has carried out an action, if they tick yes to confirm they have it passes. If they don't an error message displays that prevents them from saving the content.

My issue is that I keep getting 'illegal choice detected' when the triggering selection isn't selected.

Any help?
