Ubercart Affiliate v2, 6.x-2.6

Hopefully I'm just doing something wrong. I have two roles assigned as affiliates

referrer (for tracking)
affiliate (for pay out)

Settings on: /admin/store/affiliate/commissions

Affiliate hierarchy depth: 1
Default Affiliate Commission Structure: 0%
Affiliate Commission Structure for cookied role: affiliate: 15%
Affiliate Commission Structure for cookied role: referrer: 0%

- Order comes in on an affiliate role, but no commission is credited.
- Manually change the “Default Affiliate Commission Structure” to 10%, and “Re-apply affiliate commission” (in /admin/store/orders/NNNN) and 10% commission is credited.

So, the question is...

How do I get the affiliate role to receive the 15% and the referrer role 0%

If this has been written up (seems like it should), please just post a link to how to set this up properly. (Google is your friend until the terms are so common nothing relevant comes up.)
