Implement a hook_help() for the project

I was working with another new developer who couldn't figure out how to use this module -- he didn't see the instructions in the readme, and a hook_help will probably help people out.

I've attached a simple patch that puts the same text in the readme into a hook_help, making it accessible through the modules list.
