I am trying to upgrade my site from 6 to 7.
I have followed the instructions in upgrade.txt and the concept has worked. But I have been left with quite a few modules that are not compatible (including the theme). Do I simply reinstall the newer versions of the modules and themes and apply any changes that we had made? Or is there a way of taking the module (version 6) and running it through a process that makes it work in 7? Will all of the pages be recognised in 7? And all of the menu items?

The core upgrade will keep all your page structure, menus, blocks and aliases. You have just one thing to worry about: META tags. In D6 you probably use Nodewords module, which in D7 is replaced by Meta tags module - and no automatic upgrade between these two. So, if you want to save your description and keyword tags, you first copy them all, and then enter them manually on Meta tags module.

D6 themes and modules DO NOT work under D7. I'd advise you to start the upgrade process with your theme. It's really not difficult to convert it. This article and this article might be of some help. But even before converting the theme, make sure you have no modules that have no D7 versions that you are not willing to let go. Because if you do, you have to stay with D6 anyway.
