Some of our users get this error: This authorization code has been used

when they try to login to our site. I haven't been able to personally reproduce this issue but someone I know has. He keeps getting the error over and over and I have not once gotten this error. Anybody have insight into how to resolve this issue? I even updated to version 7.x-1.6 and still have the issue.
