I tried for quite a while to get at least a debug output of a comment form, but I failed. I believe that your module works, but it definitely lacks a lot of documentation, how to use it properly.

Watch out for white spaces at end of lines, see the end of "Optionally to use PHP in the "Set a form element" action you will need to" also on the empty line above and the line below

Sorry the last patch was a first draft and incomplete.
Here's an updated version with the excess white space removed and slightly modified instructions. I've changed added better instructions on setting up the “use PHP for settings” permission.

Thanks Andy looks good, and no big deal about the white spaces my editor always strips them out. you can also install https://drupal.org/project/dreditor and this will give you a review button next to each patch a great way to review patches in issues.
