Hi,

There is this notice Notice: Uninitialized string offset: 0 in _hs_process_render_create_new_item() on line 559 of the hierarchical_select.module on the 7.x-3.x-dev version.

There are issues for similar problems, but one is for other part of the module and the other one is for version 7.x-3.0-alpha5 and does not apply on the 7.x-3.x-dev version.

I have used the jweowu's patch to produce applicable patch for 7.x-3.x-dev. Attached in the comment below

In the last 5 years these appear to have been tidied up. If this is still found please create a new bug report with detailed steps to reproduce. Ideally starting from a clean Drupal install of the Standard profile.
