To allow for developers to override or customize the HTML generated to replace embed codes, all HTML output should be done in a theme() function. This patch moves some code from _mytube_replaceembed to a template file, which allows it to be customized using hook_preprocess_mytube_instance() or by overriding the template file.

The patch should apply cleanly to the 6.x-1.x branch.
