Add theme hook for comment_notify_disable_page function output

The comment_notify_disable_page function results in a user being unsubscribed to comment notifications on a specific node.

The function shows a message on the page with its results. This output is returned as a string. By adding a theme hook we return a template file containing this string instead. This makes it possible for themes to override this template and implement their own styling.
