Not sure what's happened but the entire address field disappears from user add form when I change the country selector from UK to Ireland.
Probably worth noting that I have multiple address fields, and I have also installed the 'Edit Profile' module.

Edit: also got this Ajax notice, not sure if it's related:

Notice: Undefined index: #ajax in ajax_form_callback() (line 379 of includes/ajax.inc).
