The drupal-org-core.make should have direct links to d.o

It is hard to quickly review the core patches done by drupal-org-core.make

Attached patch adds direct links to d.o which helps from almost any IDE to follow.

I'm curious about the needs for some patches. It would be nice to have reasoning for those in drupal-org-core.make too. But that's out of scope for this patch.
