Currently the "add" link (that's displayed below the list of current field_collection_items) doesn't have any title="…" attribute. Here's a patch to add that.
Why? I honestly don't know anymore - our UX department wanted that at some point.

What do you mean by your UX department though? Is this like a Drupal standard? A citation to a document could be helpful, or maybe somebody can look at some links in Drupal Core to see if it's usually present.

I looked around at some of the links in Bartik and I didn't see title attributes. There are more of them in Drupal 8 though so this may come up in accessibility issues for it.
