There is an issue with Drupal 6.4 and I've traced this issue back to Drupal 5.x also.
A user registers a new account. When an admin visits admin/user/user and decides to block this user (or carry out any action) this enables the "last access" column and starts counting as if this user accessed the site.
Clearly a bug. Setting to 7.x but the fix needs to be backport this to D6 and D5.
