If the Book Navigation Block is set to "Show block only on book pages", the block only appears if you are on a book page AND if the book is a child of the first item in its menu. For instance if you have your primary links set up as 

link 1
-sublink 1.1
link 2
-sublink 2.1
--sublink 2.1.1
-sublink 2.2
link 3

the block will only appear if the book is located under link 1 or sublink 1.1. It will not appear if the book is located under link 2 or 3 or any of their sublinks.
