Please forgive me if these should be separate issues.
I've installed Wysiwyg API 5.x-0.5, with the TinyMCE 2.1.3 engine, along with Image Assist 5.x-2.x-dev (and all the modules on which these depend).
I am able to view the Image Assist option in the Wysiwyg profile setup, but the button does not appear in the editor interface. The "common" TinyMCE buttons (link, bold, italics, etc.) can be added and removed just fine.
Also I am unable to realign the buttons from left to right, or center them. They are stuck on the left side, no matter what I choose. I am able to move the button bar from bottom to top of the editing screen.

sunI tested with the latest development snapshots of Wysiwyg 5.x-0.x and Image Assist 5.x-2.x and was not able to replicate this issue.

sunPlease try the following:
- Move the 'tinymce' folder from wysiwyg into a temporary location.
- Remove both the 'img_assist' and 'wysiwyg' folder.
- Download the latest development snapshots of img_assist and wysiwyg.
- Move the 'tinymce' folder back into the wysiwyg folder.
- Clear your site's cache.
- Clear your browser's cache (additionally restarting the browser won't hurt, too).
- Test again.
