When I created a Project assigned to one user (User1) and then within that create an Action assigned to another user (User2), the Project is listed under the User1's task list (that makes sense) while the Action is listed in the User2's list (also makes sense), but this Action is a sub-item of "Unknown Task".

Surely the task name should still be visible in User2's list (perhaps not as editable) if it is assigned to someone else. If you do this multiple times (i.e. assigning Projects to one user and sub-Actions to another) a user with multiple sub-Actions to projects they don't control ends up with multiple "Unknown Task" Projects.

I've attached a screen shot which I hope clarify's things.

I noticed the same problem, which resulted in my searching to see if the problem was known -- and I found this thread. Unfortunately, the problem was reported here back in January, and no action has been taken (or even an acknowledgement of the bug report). For me this indicates that this module is not being actively developed enough to warrant using it on my site. Too bad, it would really be nice to have task lists. I have raised this to critical, as this problem makes the task lists of limited use in a collaborative environment.
