The help page page does not list all the available tokens. This can lead people to think that they are not available:
admin/content/nodewords/path/tokens
My CCK field was not showing up. I then went to the pathauto help page for tokens and found my missing token there:
admin/build/path/pathauto
I then copied the cck token field name, which I had called subhead, from the pathauto help page, [field_subhead-raw], and entered it in an item on this page, and it showed up correctly in the HTML meta description:
admin/content/nodewords/path

As just like node tokens I assume the the cck tokens will probably only work on a node page and other tokens will probably only work on certain pages, so will leave it up to you to work out what tokens are going to work and be useful to you.
