Error message appears as follows, don't know why, searched using phpmyadmin and only one line exists in the system table for each respective error. Only the three said themes even exist. Any pointers, advice, caveats etc would be grateful.

Does running cron manually help ({site's domain}/admin/reports/status/run-cron)?

My installation of Drupal 6.12 returns the same error for each installed (not necessarily enabled) theme:
user warning: Duplicate entry 'themes/{theme}/{theme}.info' for key 1 query: INSERT INTO {db name} (name, owner, info, type, filename, status, throttle, bootstrap) VALUES ({values}) in {path}/modules/system/system.module on line 821”.

Aforementioned line 821 is part of function which returns an array of all available themes and their data. It consists of the following code:

What was the problem which caused the error? Or does it just disappear? If it is so, I don't think it's fixed. I have got the same error.

So maybe this issue could be used from all the other guys out there who gets this Duplicate entry warnings in modules\system\system.module on line 821. to join the efforts.

May you check if the problem occurs after cron was running (or maybe cron was failing). Are you using the admin_menu module? Does the error warnings appear when it is disabled?

The error messages disappeared, but then this happened:
1) I updated some modules and run {site's domain}/update.php
2) The messages reappeared (on {site's domain}?q=admin/reports/dblog)
3) I ran cron manually by visiting {site's domain}/admin/reports/status/run-cron
4) The messages disappeared.

Hence, it seems that this issues still needs some work indeed.

I have been getting this lately as well.

My page has been loading crazy slow, and then when I inceased the memory allowance on my PHP, it popped up listing duplicate entries for EVERYTHING on my site.

The only thing I can think of is when I recently moved some modules from [site]/modules into [site]/sites/all/modules. (Not Core modules, contributed modules that I didn't know to put them in that folder initially.)

I thought maybe, since I moved the files (leaving nothing behind), it might have recreated some settings or designations, thereby leaving me with some errors... But I have no clue.

Issue is caused by system_theme_data dropping all records from {system} and re-inserting them. If this happens while another request is being made, it causes a race condition and duplicate inserts. See #147000: Rewrite module_rebuild_cache() and system_theme_data() and #307756: Backport of #147000: Unify and rewrite module_rebuild_cache() and system_theme_data().

I tried the following steps and I don't see the errors above appearing again:

1. Update core and contrib modules
2. Go to the themes and assign a different default theme.
3. Reassign your default theme back to your usual theme.

Doing this seems to clear the theme registry.

I can report the same issue in Drupal 6.13.
Reseting themes to defaults clears the registry temporarly but after a while it comes back:
These are recurrent and found in admin/content/node-type/*

Why is this being checked on this url kind of path?
It shouldn't be looking for this ( why is is browsing the list?)

Happens to me intermittently as well. Now that I know it's not just me, I'll start keeping better track of when it happens and what I'm doing when it happens.

Here's some info about my platform (although I seem to recall it happening on a couple production sites as well, and on a WAMP install a while ago)... Mac OS X 10.5.6 running MAMP 1.7.2 (PHP 5.2.6, MySQL 5.0.41), Drupal 6.13.

I noticed in comment #7 and #14 it was mentioned that Drupal was running slow. Ever since I've experienced this error Drupal appears to be running slow, but if I run Cron.php things speed up for a short time. I'm not sure if this is a related issue or if it's just by chance but it seem strange that two other people have experienced the same thing.

I've also subscribed to #307756: Backport of #147000: Unify and rewrite module_rebuild_cache() and system_theme_data() but haven't yet applied the patch, still have a few things to test. I thought this issue might be handy to work out if the speed issue is related.

So i didn't know about placing the addon modules/themes in the /sites/all directory, never the less, the website has been running fine up until today. so i got the same error as others on this thread. I have now moved the themes and modules to the appropriate place, ran update.php and the error went away, however when i ran cron, the error came back. Has anyone had any luck figuring this out.

I'm seeing this behavior on 6.14; I'm also intermittently seeing the "user warning: Duplicate entry..." behavior where there's a warning generated for every module, content type and theme on the website.

I have not seen this error prior to 6.14; I went straight from 6.10 to 6.14 on most of the sites I'm working on, so it's certainly possible I've missed it before, but this is more or less crippling. When it happens, it takes the site to a standstill -- I believe, but don't have a way to tell, that when these errors are being generated it pegs the MySQL process, which effectively hangs the site until all errors have been generated. Hundreds of errors are generated, and it appears to be precipitated by flushing the caches.

I got today the same error msg (that with duplicate key). Don't know what was the source of it, neither I could reproduce it. Error was reported for every theme I had within drupal maptree, installed or not. Just to let you know guys that this is still happening. My site stil works without any problems, but I was really concerned until I saw that many users reported that this error didn't affect their site integrity. I use drupal 6.14

I get the same thing on a fresh drupal install. All I can say is how's that advanced CMS thing working out for us ? Wordpress is looking mighty fine these days. I get tired of all these unexpected drupal warnings and error messages, it's non-productive. Site owners should not have to deal with this. D7 is looking good though who knows how long until it gets released and when it does there will another waiting period for D7 modules just like D6, could be another year until it's usable.

If I understand issue #251792: Implement a locking framework for long operations correct, this is already fixed in -dev and will be included in 6.16, from comment #296:

this patch is already a part of 6.x-dev and will be a part of 6.16 whenever that is released. So you can either patch your existing 6.15 installation, switch to 6.x-dev, or wait till 6.16. Either way remember to run update.php.

WHOA.... Huge clue here?

I am indexing 500 records per CRON run, and my cron fires off every minute, lasting about 15 seconds. The freakout described above happens ONLY AND ALWAYS during the cron job execution.

Oooof, I'm embarrased I didn't notice this sooner... It's happening not just when my cron job runs, but any two concurrent requests including two anonymous users.

It will be rather embarrasing if it happens 100% of the time I receive concurrent requests - any developments here?

For the record, I using role-based theme switching between Acquia Prosper+Fusion Core+Skinr and CTI Flex+Zen...

This happens to me every time I open a bunch of tabs at once. If I just browse "normally" it doesn't happen but as soon as I open 2 or more Drupal links in new tabs, it happens. (I'm usually in admin when I'm doing this - if that helps)

Same happens with 6.16. I'm not using skinr.

I was running browsershots.org at my site to get a bunch of screen images from different browsers. Maybe that caused it? Only got an error message when not logged in. Running cron.php solved the issue.

I'm on D1.16 and it looks like upgrading Skinr from v1.4 to v1.5 solved this issue for me.

I'm just another victim with the same problem. The error is occuring on a regular base, but doesn't seem fatal in my case. This seems to be the same deal with the other comments posted here.
I don't think this has anything to do with the skinr module, because i've never used it. But some of you are saying it can be fixed with it? or was that a false alarm?

Also i get the error once for each theme i have available (enabled, disabled or default doesn't matter).

I've disabled and uninstalled the devel and theme developer modules, and i haven't got the error since.

I've only installed "Administration Menu" and "Backup and Migrate" modules.
I didn't create any content, roles, or users. I only configure 'Backup and migrate' module to make scheduled backups (using cron, of course).

The error is triggered at the same time that cron starts, and executes the backup of the site. I run cron 2 times a day, and I do a scheduled backup once a day

Was running Drupal 6.16 and upgraded to 6.17 then started getting error messages similar to these, unpredictably, and my site slowed quite a bit...

So I re-copied all of the Drupal core files to my directories, thinking that something was corrupted, flushed the cache and ran the cron, but it didn't go away.

Next I updated all of my modules and made sure there was no other related errors in my dblog, still the site ran slow.

On a whim I checked to make sure I was using the correct theme and that everything looked alright, to my surprise I found that my theme was not selected.
What I mean is my theme had the "default" radio button selected next to it, but some how the theme enabled check box had been unselected. One would
figure that this should not matter much because the "default" should automatically be enabled if nothing is selected, but I guess that this is not the case.

It is possible that I forgot to re-check it after reinstalling the core files, but doubtful, and regardless it still appeared as if my theme was working properly.

Well after re-checking the box, clearing my cache and running cron again, things seem to be working, fast and error free, at least for the time being.
I would only assume that running the site with only "default" selected does not ensure that all parts of your theme are active. If this is the case either the default button is useless or is malfunctioning.

Anyway, hope this helps someone.

yes, sometimes I get these enormous error list and normally goes away if I reload one or two times.

But now I have some more themes installed and the enormous error list remains on reloading. Yes, same as some months ago; the themes are ticked off by Drupal errors (not by humans).

And this time I only had one theme selected that i really need, because I knew from months ago this part of Drupal doesn't work well.

Still predetermined is ticked on, but "active" was ticked of by this Drupal error. Next to this error, it happens just too often that this error list returns and on reload disappears without drupal errors ticking themes off.

Still seeing this problem, and it causes a big slowdown when it is erroring out and then renders an enormous page full of errors.

same here. I think this is a really critical error. I also got this error in other up to date drupal websites. So i wonder why not everyone with the same version of drupal (6.17) has the same problem.

Just had this re-occur and cripple one of my sites. I was using a Fusion-based theme previously, and had been running Skinr, but had switched to a simpler theme months ago, and this issue just suddenly reappeared. Trying to clear it out now, without much success.

I had this problem after moving modules from drupal/sites/site.com/modules to drupal/sites/all/modules.

Flushing all the caches worked for me.

I had a rogue piece of code within a custom module - removed this and viola done.

was something like this within a form array :

Has anyone found a simple fix? Im using DV 6.13. My "Warning: Duplicate entry 4842 for key PRIMARY......." in on the home page of the site. Need a fix fast.

It's the same error for every theme drupal can find. That's why in most cases the error is so huge. By removing all the themes you don't use (also core themes u don't use), the error shouldn't take in the entire screen anymore.

I also haven't got this error since i removed all the unused themes. Keeping my fingers crossed.

Understood, but in the other threads there is also still no clear explantion or solution for "end-users" to be found imho.

This will be thread where users running into these error messages will end up when searching with Google or on this website. And the long list of patches and technical comments about core code/functions on the other post are a little demotivating for non-developers I guess.

All themes turns off after error appearance. Status become "0" for all themes in system table.

ThemeKey Debug Messages: A matching Theme Switching Rule to select theme [theme_name] was not applied because this theme is disabled.

I'm having the same problem here with 6.20.
I upload a module (extlink, multiblock, ckeditor, imce, etc.) and then after I enable it, I get the error. When I disable, it stops.

Some modules (ckeditor and imagecache, for instance) didn't make the error when I tried for the 3rd or 4th time to enable.

Also I get the error when manually run cron.

Could this be due to importing translations?
I noticed that when I enable and the disable (because of this problem) a module, the translations are imported and maybe the reason for the duplication?
I don't know if this makes any sense.

In my case, I started getting this Warning Message regularly today only after updating unrelated modules via "Update"... pretty much every couple admin pages I went to I'd see this long warning message... Refreshed Cache, Ran cron.. but to no avail. I've seen this warning message for a year now, and I'd almost guess it started a little after installing Admin Menu

But it got real bad today... In going to Account Edit, I used to be able to change Theme Configuration. Today, I couldn't even get that far.. by the time I went to Account Edit, the theme configuration wouldn't appear because immediately after going to User/sally/edit, the theme settings at admin/build/themes would automatically disable. Thus, I'd go back there and see that my garland theme default bubble was the only thing left selected.

My only solution as of now is to disable Admin Menu

Update: My site is going bananas as a result of trying to rid this error.. nothing has been resolved.

I'm seeing this error in 6.20 as well. Disabling Admin Menu did not solve the issue. Looks like it's time to disable all modules and see which one makes it break, one at a time.

UPDATE: For me, the culprit seems to be the Country Select module (http://drupal.org/project/country_select). Turning just that module rids me of the errors.

It seems to me the problem is speed of database response. Hosting service that I used got a problems with large iowait and messages about duplicate entries become permanent. I switched to another service and forget about this messages.

I also get the problem intermittently. Refreshing the page where I see the "user warning" makes it go away so I've tended to ignore it :-) Today I read through the early comments in this issue and noticed the cron hadn't run for over 24 hours, so I ran it manually and either get "cron run failed" or 500 internal server error :-/

I had the same problem, none of the above methods work so i did the following in the system.module on line 822, i'm not sure how it will affect the site but it got rid of the problem, mine was caused by flushing cache--> using some sort of ajax query (in my case uc_out_of_stock module) and then redirecting to any page on my site showed the error, can anyone tell me how the following change will affect the site?

Hi Desi Raaj,

I applied your fix in #115 about a week ago. The errors have since gone away... I was getting them intermittently once or twice on average every day on my production site.

But now, my enabled page themes have been breaking so that my admin theme reverts to default theme when I do not want it to.

#1. As far as I can tell, if I disable Admin Menu my problem goes away.
#2. I find Admin Menu very useful and want to keep it.
#3. The error is more persistent than it has ever been.
#4. At this point, I'm not sure if my error is the same as everyone else's.... but it looks pretty close.

In any case, if I follow your link I see your comments, however, you also posted a patch at http://drupal.org/node/894710#comment-4062442

Would you explain to a dummy who has this issue... I don't know what you are talking about in #21 because I'm still Drupal illiterate.. BUT... Should I:

A) figure out your solution in #21 ( http://drupal.org/node/219910#comment-4063656 )
B) or apply rjb's solution in #22
C) or apply your original patch and see if that works?

Thank you. If I figure this out I'll outline a solution for other newbies to use.

EDIT: I'm with Candelas below: I dumped Admin_Menu so now my problems are resolved. Maintainer believes I installed Admin_menu but didn't update it after installation (I don't think that's the case as I've run update countless times and have reinstalled module update). Or maybe it's acquia's distributions he says... Either way, I have to move on. Can't resolve this so am quitting admin_menu module.

EDIT: After uninstalling Admin_Menu and enabling Simple Menu module for first time, I encounter the same issue: Theme breaks constantly. My admin account --Garland theme breaks... I sometimes I get visible errors on user theme: SKY.. Upon going to Themes, I see only the original default theme is still enabled, however, its default option is now disabled. TO DO: Uninstalling Simple Menu module

Update: Problem persists even after uninstalling Simple Menu module. HOWEVER, Warning Errors are not visible as they were before when using Admin_menu. But show up in Recent log entries in admin/reports/dblog

August 28, 2011 Update: In Log entries, I noticed a lot of PAGE NOT FOUND notices. Something like: "PAGE NOT FOUND: sites/all/themes/worksheet/images/none.png" -- This was a result of having deleted an image in my themes images folder or messing with the CSS file itself--in any case the file doesn't exist. This resulted in some of the Duplicate Errors. However, after 'cleaning' my theme files I was still getting the Duplicate Error every so often after Cron ran. As a result I thought since it's theme related I'll disable Page Theme and see what happens. Since disabling PAGE THEME module and replacing it with SECTIONS module, I am Duplicate Error free. (Note: have not reinstalled Admin Menu module yet) PROBLEM PERSISTS

i disable admin menu, and my problems are gone. no need of patches.
an alternative is http://drupal.org/project/simplemenu

Like #80, i noticed that in my theme settings, the theme that had the default radio checked, was not enabled. I started having this error after enabling and disabling different themes, and then I got to the state that my default theme was not enabled. Everything seemed to work, but after a while i got these errors. Now they are gone, so I thought someone else might have the same issue.

What you just described happens to me on a daily basis. I re-check my "enabled" boxes constantly, only to see them disappear.. what's left is the default radio checked. I don't know what to do at this point. The user, once in a while, maybe a couple times a week will refresh a page and get hundreds of Duplicate Errors. This site just can't go live until I figure this out. This has been going on for 2 years. I've updated all my drupal installations. Uninstalled modules I thought may be the culprit. The only thing I know now is that my Mac Laptop, with the same installation doesn't get the errors as frequently, or at all. I'm using a Mac OSX desktop, leopard. Also, I'm thinking maybe Acquia Drupal might have something to do with it. Or the way it's loaded. I'm going to uninstall everything and start all over. But if I do that, should I go module by module? Sometimes I have to wait 30 minutes to be editing my site before I get the first errors. It will take weeks for me to figure out just how to avoid getting the errors.

I have this problem on 6.22, for example on the page admin/content/comment: user warning: Duplicate entry 'themes/garland/minnelli/minnelli.info' for key 1 query: INSERT INTO system... etc. The long threads about this without any apparent resolution are quite bewildering. I thought this error message only appeared to authenticated users but looking at google search, there are loads of recent examples of this error message showing for anonymous users, for example:
