If the default language isn't English and English has no path prefix assigned, the url alias always fails for English pages. (Add 'en' prefix, if English isn't default language?) (Irrespective of language select mode.)

I am having the same issue.
I turned off pathauto for now.
I had a language that was not English as the default one.
All aliases had correct language prefix added, except English.

Default language: English
Second language: German
Third language: French
The Autoalias setting works for English and German but not for French.
It isn't clear if this is a problem for Pathauto, I18n or both.
