Hi. I'm maintainer of the profile_pictures module and I want to integrate with this module (due to #786176: Doesn't work with One Page Profile or Profile Privacy).
I think that calling drupal_alter for profile_form_profile is the better solution. It also will be useful for integration with profile_privacy (instead of available spike) and other core profile modules.
