We are migrating from simplenews to mailchimp but have most of the site subscribers already in mailchimp.
When this module is activated and setup how does it work? Will it sync subscribers already on mailchimp?
When a user signs up do they get immediately added to mailchimp or is this done via cron?

It only pushed data/users from a Drupal site to Mailchimp, not vice versa. That said, it can automatically push an existing Drupal user base into one or more MC lists.

There's a setting allowing it work either way.

When you say it can, how does this work? How is it configured.
When it pushes the existing user base, will it or mailchimp handle duplication?

You map a role to an MC list and mark it as required.
