Fresh Install - Problem on Database configuration screen

I am installing Drupal for the first time, however i have followed the documentation install instruction, and got to the stage of Database configuration. I am stock at the screen because after putting in db name, db user name and pw, it returns with no error but same screen with blank db name, db user and pw screen.

Please any one know what the problem is. Need your help urgently.

The db name, user name and pw exit in MYSQL and the proper rights is granted.
