Hopefully I'll save someone some headaches with this report. If a files fails to upload (my cause was exceeding the max upload limit), then the user just gets redirected back to the upload page with no indication that something failed. I eventually had a eureka moment and realized that might be the issue, increased the limits and then tried again ... everything works beautifully. It would be really nice to give a user more feedback about that ... I feared that the module just wasn't working, but it is really, really nice now that I have that resolved.

I can confirm that this is still an issue. However, I noticed the same issue with the file field in Drupal core (when not clicking the "Upload" button, but instead clicking only the "Save" button on a node form). The difference with the Drupal core file field is that it also reports the maximum file size in the description of the field, where Feeds does not.
