When I attempt to duplicate a book page (with several levels of subpages) with content as 1 copy, I get two copies. One with the subpages, and one without. Let me know if I can get you any more info.

have you tried just installing on stock drupal?

is this localhost or web server?

What other JS based modules do you have installed?

Are you using PURL?

Are all the types being created able to be put in books?

If you refresh the page are they ordered correctly?

What version of Drupal are you using?

What other outline designer / book management projects are you using?

Clean URLs or not?
