When swapping to the secondary country the province box appears underneath all fields in the category.

I believe that this may be due to the weight of the field or possibly just that it is added to the form last.

This issue has been fixed in release 6.x-1.7. (Required another workaround caused by the poor implementation of the core Profile module.)
