overhaul "More options" functionality

Description

I need to set up a voting situation where there will be more than 5 options to choose from. The module allows me to enter the additional items, but when the poll is viewed, there are still only five options.

We should take the work done on advpoll for 'more choices' that is AJAX-based instead of our 'click + preview' system.
