I read in ds issues that Rel can register any form.
I want to use ds forms on my entity form page.
Is it possible to do this with rel , to use ds form templates on entity forms?

It should be yeah, the latest release of display suite has a submodule called ds_forms so you can use templates as well.

By default ds forms can only be used nodes pages.
Event the latest stable version has ds_forms
