Just thought i'd make some suggestions to tighten up the actual forward form:

1. The 'send to' box - it doesn't really need to be multi-line does it? Surely it can just be one line and addresses separated by commas. Also that should stop quicktags showing up above it which you don't need in email addresses.
2. In the 'you are going to email the following' bit - would be good to replace (Your Name) with the actual name, no?

3. Subject line should be able to have the title of the page being forwarded.

Item 1 is fixed

Item 2 is actually not possible, since we don't yet know the name of the user until they actually submit the form. Also, the line you mentioned above that is referring to the page title which was missing because of another bug (also fixed).

Item 3 is probably a matter of preference. I think the best solution will be to add another wildcard for that to the list in the admin settings. That way you can change it to show the title if that is your preference. I'll have to do that later as I'm short on time at the moment.
