Clicking on Manage Form; Window keeps trying to load but never does.

Hello,

I'm using Drupal 7.12. I enabled the module and also enabled form registration. However when I click the manage form link, the popup keeps trying to load but doesn't. I have attached a screenshot.

I am a drupal newbie and from the screencast, I see this is really a great module. I will appreciate any help I can get with this issue.
