Clicking on the "Report this link as dead" link gets me to the report form, but when I enter a reason and submit, the blank report form re-appears. No confirmation message, or error message. The link doesn't appear in the moderation tab, or the dead links tab.

Same here. I looked at the code, but couldn't see anything wrong. I checked it against similar code used to suggest a link, but nothing looked odd...but I'm no expert.

Yes, I noticed this while I was testing it. I may fix this one before I have you convert to the Web Links module.
