With Javascript turned off the module works fine, I can click on "add more choices" and it adds them properly. With Javascript enabled I get the rather uninformative error when clicking on the "add more choices" button.

I have Drupal 6.24 installed, and a bunch of other modules, though nothing super wacky.

I'm happy to provide more information about my install or the problem, but I'm at a loss on where to move forward on trouble shooting here. I did disable everything on the performance page and cleared my cache.
