submission email does not contain submission values

When i submit the form, the email looks like this:
ps - i turned on debug in case it will help. The problem occurs with/without debug turned on.

If I actually go to the link in the email, I can actually see the values I entered in the form. It seems as though its just not being sent via email.

Also the subject and from address are blank although it there are values set for it.

This bug happens with version 5.x-1.4 and 5.x-1.4
