outline_designer 7.x-2.0-alpha2

Book logic abstracted from OD core and enhancements from 6.x-2.x branch pushed forward.

Bug Fixes

• #1543278: WSOD after enabling -- Outline Child Pages now works
• #1512230: Error occurs at /admin/content/book/outline_designer -- book / global settings forms now functional
• Cleaned up some ajax calls
• Duplicate should now be functional
• Add new book button is back on the book content overview page
• Requirement of a page in a book to start using outline designer now works correctly

Known issues still

• table reloading still throws "book modified by another user" errors
• all operation are successful but don't refresh screen
• titles stack on refresh of interface (its like the drupal core ajax replace function isn't working)
• OD Book settings page isn't loading the checkbox grid to hide buttons correctly
