When a menu item is deleted from the {menu} table, {menu_per_role} table doesnt get updates and does not remove old menu ID.

it would be great if menu_per_role.module could keep it's table clean and up to date with only existing menu IDs

This is 3 years old... I'm closing it. Hopefully you'll be able to upgrade to 6.x and get that resolved. There are no 5.x maintainers.
