If you set up a workflow to publish a new revision when the node is 1st created, it bypasses pathauto. The workaround is to save the node 1st, but that seems a bit hack-ish, considering that the node is also saved when publishing the new revision. A better fix would be to check $node->is_new before setting pathauto_perform_alias to FALSE.
