Visiting /file/add/foo produces the following error:

Recoverable fatal error: Argument 3 passed to file_entity_add_upload() must be an array, string given in file_entity_add_upload() (line 38 of drupal/sites/all/modules/file_entity/file_entity.pages.inc).

I found this by accident, a third argument to this URL isn't expected in normal use. Still, it's an easy fix.
