under Administer >> Site building >> Menus, the sub-nav looks like:
* Navigation (overview)
* Primary links (overview)
* Secondary links (overview)
Should be just:
* Navigation
* Primary links
* Secondary links
... (overview) should only be added to the actual page title @ http://localhost/head/admin/build/menu-customize/X
When clicking "Add item", the navigation block no longer marks Menu as the active menu item, so the sub-nav is lost. The breadcrumb is similarly messed up; it only goes to Home > Administer > Site building, so there is effectively no way to get back to the parent menu from this page.
