I have implemented most of the code to use update.php to upgrade to the newest version of the quiz module from version 1.1.

As of right now these are the known issues:
Score is not recalculated
Answers are not decoded from old format to new format but are not deleted so when someone writes a decoder, it can be converted to the new system.

What this does not do:
delete any necessary information which is needed for upgrade to the quiz head version.
convert your scores to the new system (maybe kscheirer can do this)
any converting from serialized to non serialized format. If it's serialized in 1.1 it isn't changed to the new schema.

Fixed the score, so that it is changed to is_correct in quiz_multichoice_answers.

I have cleaned up and rerolled the old patch, for the newest version of quiz module. No other changes, aside from minor things to the documentation.
